This study aimed to identify what qualities and characteristics of knowledge and comfort would affect a speech-language pathologist's decision to recommend/not recommend a free water protocol as part of a dysphagia patient's plan of care. The knowledge and comfort level of medical personnel of the Frazier Free Water Protocol was analyzed via a survey distributed to 84 speech-language pathologists and 31 nurses. Results suggested that speech-language pathologists are more inclined to consider implementing a free water protocol to patients who are provided with appropriate oral care, and cognitively intact, and compliant/cognizant to the protocol and its consequences.

Patients with swallowing disorders are often put on diets restricted to thickened liquids. Thickened liquids are used for patients who often aspirate on thin liquids. Patients may not comply with drinking thickened liquids and at times may decrease liquid intake, which could cause dehydration. Dehydration is common among the geriatric population and has contributed to high rates of morbidity and mortality (Panther, 2005). In 1984, the Frazier Rehabilitation Center in Louisville, Kentucky changed their swallowing management protocol to what is now known as the Frazier Free Water Protocol (Panther, 2005). The protocol was put into effect to help patients become more compliant with the restriction of thin liquids and also to decrease the occurence of dehydration. This study is designed to investigate the practices and perceptions of speech-language pathologists in Kentucky regarding water protocols for dysphagia management.
