Disease Control Priorities, Third Edition (Volume 2)

The evaluation of reproductive, maternal, newborn, and child health (RMNCH) by the Disease Control Priorities, Third Edition (DCP3) focuses on maternal conditions, childhood illness, and malnutrition. Specifically, the chapters address acute illness and undernutrition in children, principally under age 5. It also covers maternal mortality, morbidity, stillbirth, and influences to pregnancy and pre-pregnancy. Volume 3 focuses on developments since the publication of DCP2 and will also include the transition to older childhood, in particular, the overlap and commonality with the child development volume. The DCP3 evaluation of these conditions produced three key findings: 1. There is significant difficulty in measuring the burden of key conditions such as unintended pregnancy, unsafe abortion, nonsexually transmitted infections, infertility, and violence against women. 2. Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.




Plant-produced Microbial Vaccines


Book Description

In recent years, plants have been increasingly explored for production of biomedicines and vaccine components. The two main advantages of plant systems are low cost and a greater potential for scalability as compared to microbial or animal systems. An additional advantage from the public health point of view is high safety compared to animal systems, which is important for vaccine production: there are no known plant pathogens capable of replicating in animals, and in humans in particular. A particular antigen or a protein has to be expressed in a plant using one of many available platforms; this antigen/protein subsequently needs to be purified or processed, and later formulated into a vaccine or a therapeutic; these need to be delivered to a human or animal body via an appropriate route. Naturally, all these vaccines and therapeutics must be subjected to regulatory approvals prior to their use. Thus, the challenge is to adapt plant-based platforms for production of cost-efficient biomedicals that can be approved by FDA for use as vaccine components or therapeutics which will be competitive against existing vaccines and drugs. This volume attempts to address the entire spectrum of challenges facing the nascent field of plant-based biomedicals, from the selection of an appropriate production platform to specific methods of downstream processing and regulatory approval issues.




The Critical Public Health Value of Vaccines

Immunization against disease is among the most successful global health efforts of the modern era, and substantial gains in vaccination coverage rates have been achieved worldwide. However, that progress has stagnated in recent years, leaving an estimated 20 million children worldwide either undervaccinated or completely unvaccinated. The determinants of vaccination uptake are complex, mutable, and context specific. A primary driver is vaccine hesitancy - defined as a "delay in acceptance or refusal of vaccines despite availability of vaccination services". The majority of vaccine-hesitant people fall somewhere on a spectrum from vaccine acceptance to vaccine denial. Vaccine uptake is also hampered by socioeconomic or structural barriers to access. On August 17-20, 2020, the Forum on Microbial Threats at the National Academies of Sciences, Engineering, and Medicine held a 4-day virtual workshop titled The Critical Public Health Value of Vaccines: Tackling Issues of Access and Hesitancy. The workshop focused on two main areas (vaccine access and vaccine confidence) and gave particular consideration to health systems, research opportunities, communication strategies, and policies that could be considered to address access, perception, attitudes, and behaviors toward vaccination. This publication summarizes the presentations and discussion of the workshop.

New-generation vaccines and novel vaccinal strategies against infectious diseases of livestock, wild and companion animals

Emerging and re-emerging infections, in particular those caused by viruses, are expected to rise in correlation, among other factors, with climate changes. Antibiotic resistance is another issue that will limit the therapeutic arsenal against bacterial and parasitic infections. Therefore, one must be adequately prepared to overcome and prevent current and novel infections, and vaccination remains the optimal way to fight infectious diseases in humans. This is also true for many impacting diseases of livestock and companion animals, to which there are no available vaccines. First (attenuated and inactivated) and second (subunit) generation vaccines have their limitations. To overcome this, efforts have been made in recent years to develop novel vaccine types such as virus-like particle (VLP)-, viral vector-, and DNA-based vaccines in humans and animals. The recent success in utilizing RNA technology based-vaccines to fight SARS-CoV-2 to limit the severity of clinical symptoms and mortality constitutes a promising way to develop such vaccines against animal diseases. Development of novel molecules or materials with adjuvant or immunostimulatory activity to adequately stimulate the innate immune response, and, therefore the pathogen-specific adaptative immune response, as well as the identification of suitable antigenic material that would bring broad protection against infectious agent variants are other key issues to be considered in developing safe and innovative immunization strategies.
