The initial eight chapters provide a synthesis of the wide variety of treatments currently available for malocclusions in the sagittal, transverse, and vertical dimensions. The subsequent chapters describe the details of clinical management of fixed appliance therapy (e.g., bracket placement and design - including self-ligating brackets, orthodontic bonding, archwire sequencing, transpalatal arch and utility arch management). Dentofacial orthopedics, including rapid maxillary expansion and extraoral traction, as well as functional jaw orthopedics (Herbst, Twin Block, FR-2 of Fränkel, Bionator) and molar distalizing appliances (Pendulum, Distal Jet, Wilson distalizing arch) then are considered. The management of Class III malocclusion with the orthopedic facial mask, the chin cup and the FR-3 of Fränkel also are discussed in detail. The clinical and cephalometric evaluation of the orthodontic patient and the relationship of orthodontic treatment, occlusion, and temporomandibular disorders also are presented.
