A Content Analysis of the Utilization of Neurologic Music Therapy

There is a need for evidence-based research in the field of music therapy, and interest in evidence-based practice among music therapists in the United States has led to the development of neuroscience informed approaches to music therapy. Neurologic Music Therapy (NMT) is a specialized application of music therapy that applies research from neuroscience to evaluate what occurs in the brain as music is being perceived or produced and how it influences non-musical behavior and function in order to provide effective therapeutic outcomes for people undergoing neurologic rehabilitation. NMT is a system of twenty therapeutic techniques categorized by the various functions of the brain they were designed to address into three domains: cognitive, sensorimotor, and speech/language. The techniques include Musical Sensory Orientation Training (MSOT), Musical Neglect Training (MNT), Auditory Perception Training (APT), Musical Attention Control Training (MACT), Associative Mood and Memory Training (AMMT), Musical Executive Function Training (MEFT), Musical Mnemonics Training (MMT), Rhythmic Auditory Stimulation (RAS), Pattern Sensory Enhancement (PSE), Therapeutic Instrument Playing (TIMP), Oral Motor and Respiratory Exercises, (OMREX) Musical Speech Stimulation (MUSTIM), Melodic Intonation Therapy (MIT), Rhythmic Speech Cueing (RSC), Vocal Intonation Therapy (VIT), Therapeutic Singing (TS), Symbolic Communication Training Through Music (SYCOM), and Developmental Speech and Language Training Through Music (DSLM). The purpose of this study was to investigate if there has been an increase over time of populations and diagnoses for which neurologic music therapy is utilized to target areas of focus for future researchers. A content analysis of 632 journal articles published in scholarly music therapy journals was conducted and 22 articles were identified for further inspection. Overall, both the age groups and variety and breadth of diagnoses of participants included in studies has increased over time. This suggests that music therapists are actively researching and utilizing NMT, which is vital given that the burden of neurological disorders has risen since 1990. Further research is needed to continue expanding our knowledge of how and for whom NMT can best serve as a successful music therapy approach.

Music is an important source of enjoyment, learning, and well-being in life as well as a rich, powerful, and versatile stimulus for the brain. With the advance of modern neuroimaging techniques during the past decades, we are now beginning to understand better what goes on in the healthy brain when we hear, play, think, and feel music and how the structure and function of the brain can change as a result of musical training and expertise. For more than a century, music has also been studied in the field of neurology where the focus has mostly been on musical deficits and symptoms caused by neurological illness (e.g., amusia, musicogenic epilepsy) or on occupational diseases of professional musicians (e.g., focal dystonia, hearing loss). Recently, however, there has been increasing interest and progress also in adopting music as a therapeutic tool in neurological rehabilitation, and many novel music-based rehabilitation methods have been developed to facilitate motor, cognitive, emotional, and social functioning of infants, children and adults suffering from a debilitating neurological illness or disorder. Traditionally, the fields of music neuroscience and music therapy have progressed rather independently, but they are now beginning to integrate and merge in clinical neurology, providing novel and important information about how music is processed in the damaged or abnormal brain, how structural and functional recovery of the brain can be enhanced by music-based rehabilitation methods, and what neural mechanisms underlie the therapeutic effects of music. Ideally, this information can be used to better understand how and why music works in rehabilitation and to develop more effective music-based applications that can be targeted and tailored towards individual rehabilitation needs. The aim of this Research Topic is to bring together research across multiple disciplines with a special focus on music, brain, and neurological rehabilitation.
