The future of disability in America will depend on how well the U.S. prepares for and manages the demographic, fiscal, and technological developments that will unfold during the next two to three decades. Building upon two prior studies from the Institute of Medicine (the 1991 Institute of Medicine's report Disability in America and the 1997 report Enabling America), The Future of Disability in America examines both progress and concerns about continuing barriers that limit the independence, productivity, and participation in community life of people with disabilities. The United States is in the midst of a major demographic shift. In the coming decades, people aged 65 and over will make up an increasingly large percentage of the population: The ratio of people aged 65+ to people aged 20-64 will rise by 80%. By one reading, things look pretty good for Americans today: the country is richer than ever before and the unemployment rate is down by half since the Great Recession—lower today, in fact, than for most of the postwar era. But a closer look shows that something is going seriously wrong. This is the collapse of work—most especially among America’s men. Nicholas Eberstadt, a political economist who holds the Henry Wendt Chair in Political Economy at the American Enterprise Institute, shows that while “unemployment” has gone down, America’s work rate is also lower today than a generation ago—and that the work rate for US men has been spiraling downward for half a century. Astonishingly, the work rate for American males aged twenty-five to fifty-four—or “men of prime working age”—was actually slightly lower in 2015 than it had been in 1940: before the War, and at the tail end of the Great Depression. Today, nearly one in six prime working age men has no paid work at all—and nearly one in eight is out of the labor force entirely, neither working nor even looking for work. When children and adults apply for disability benefits and claim that a visual impairment has limited their ability to function, the U.S. Social Security Administration (SSA) is required to determine their eligibility. To ensure that these determinations are made fairly and consistently, SSA has developed criteria for eligibility and a process for assessing each claimant against the criteria. Visual Impairments: Determining Eligibility for Social Security Benefits examines SSA's methods of determining disability for people with visual impairments, recommends changes that could be made now to improve the process and the outcomes, and identifies research needed to develop improved methods for the future. The report assesses tests of visual function, including visual acuity and visual fields whether visual impairments could be measured directly through visual task performance or other means of assessing disability. These other means include job analysis databases, which include information on the importance of vision to job tasks or skills, and measures of health-related quality of life, which take a person-centered approach to assessing visual function testing of infants and children, which differs in important ways from standard adult tests.
