Because skin blisters are the initial manifestation of epidermolysis bullosa (EB), patients invariably present to the dermatologist for diagnosis and treatment. However, EB is a systemic disease whose management requires input from clinicians in virtually all fields of medicine, including pediatri cians, surgeons, dentists, gastroenterologists, hematologists, otorhinolaryn gologists, dietitians, and physical therapists, to name a few. Because EB is a rare disease, few clinicians are familiar with it, and many recoil at the pros pect of caring for individuals covered with blisters caused by a disease they know little about. For patients, insult is thus added to injury and they feel abandoned, neglected, and frustrated. One way to remedy this deplorable situation is to provide clinicians with a compact source of information de tailing the principles of EB diagnosis and treatment. This text seeks to fulfill this role. From 1986-1991, The Rockefeller University Hospital has been the co ordinating center of the National EB Registry. Supported by The National Institutes of Health, this Registry consists of four university centers* commit ted to collecting clinical data concerning diagnosis, treatment, and epidemio logy on all American EB patients. As of April 1992, nearly 1,799 EB patients have enrolled nationwide. The Registry is now in its second five-year phase of operation.

The endothelium, a monolayer of endothelial cells, constitutes the inner cellular lining of the blood vessels (arteries, veins and capillaries) and the lymphatic system, and therefore is in direct contact with the blood/lymph and the circulating cells. The endothelium is a major player in the control of blood fluidity, platelet aggregation and vascular tone, a major actor in the regulation of immunology, inflammation and angiogenesis, and an important metabolizing and an endocrine organ. Endothelial cells controls vascular tone, and thereby blood flow, by synthesizing and releasing relaxing and contracting factors such as nitric oxide, metabolites of arachidonic acid via the cyclooxygenases, lipoxygenases and cytochrome P450 pathways, various peptides (endothelin, urotensin, CNP, adrenomedullin, etc.), adenosine, purines, reactive oxygen species and so on. Additionally, endothelial ectoenzymes are required steps in the generation of vasoactive hormones such as angiotensin II. An endothelial dysfunction linked to an imbalance in the synthesis and/or the release of these various endothelial factors may explain the initiation of cardiovascular pathologies (from hypertension to atherosclerosis) or their development and perpetuation. The integument plays an important role in the survival of meta zoans by separating and protecting them from a hostile environ ment. Its function ranges from protection against injury and in fection, participation in the regulation of body temperature and water balance, to respiratory activity, monitoring of the environ ment and production of signals related to behaviour. All these result from specific structural, biochemical and physiological properties of intra-and extracellular components of the integu ment. Thus its characterization can be best accomplished by a multidisciplinary approach with authors specialized in different fields of science. This multi-author book, in two volumes, provides an up-to date survey of the literature. The first volume deals with the integument of invertebrates, the second with that of vertebrates, both organized primarily on a phylum basis. As the level of knowledge on the integument of phyla differs considerably, the information provided is correspondingly either limited or con densed. For some of the smaller groups of invertebrates little information is available, as often only a few electron micrographs are to be found in the literature ; on the other hand, from the large body of knowledge existing for vertebrates, particularly for mammals, no complete overview can be provided, but publica tions giving access to further information have been reviewed critically.
