The National Childhood Encephalopathy Study


Book Description

This book presents the findings from an extensive follow-up report of 1,182 children from the National Childhood Encephalopathy Study conducted in Britain. Each of these children had an early neurological illness, and each was matched according to age, sex, and geographical location with two control children. For the present follow-up, the researchers evaluated the survival, development, and capabilities of 80% of the children from the original study. They found that the children's outcomes varied considerably: while some appeared fully recovered, others were severely impaired. The size of the study enabled the researchers to identify those factors that most reliably predicted outcome, such as early diagnosis, continuing convulsions, and age at onset of the illness, as well as those factors that did not prove predictive of outcome, such as sex and social conditions. The researchers also examined the question of whether or not early neurological illness predicted a single syndrome of subsequent impairment. They found that, on the contrary, the children displayed different patterns of longer-term difficulties. This is a special issue of Developmental Medicine and Child Neurology.

Acute viral encephalitis is a rare but devastating complication of common virus infections. Primary viral encephalitis is due to brain invasion by the virus and subsequent replication in the central nervous system, whereas postinfectious encephalitis is thought to be immune-mediated. The most frequent causes of primary encephalitis worldwide are rabies and the arboviruses, which comprise different families of viruses distributed according to geography and seasonality. Herpes simplex virus (HSV) is the commonest cause of encephalitis in Western countries. Herpes simplex encephalitis (HSE) occurs at all ages during infancy and childhood with a peak incidence during the first year of life. Its pathogenesis remains unsolved, although defects in the innate immune response have been observed in a few cases. Direct and sensitive virological diagnosis is available, but may be difficult at early stages of the disease. Specific antiviral therapy has proven to improve the prognosis of HSE dramatically, if administered early. Despite use of antiviral drugs, sequelae remain high and relapses may occur, presumably secondary to HSV latency in the brain. In addition, deleterious host inflammatory responses might account for failure of therapy. Immunomodulatory treatments have been proposed but require a better understanding of the respective roles of the multiple pathways of the immune response.

Encephalitis Lethargica

Encephalitis lethargica (‘sleeping sickness’) was a mysterious disorder that swept the world in the decade following the First World War, before disappearing without its cause having been identified. Around 85% of its victims, predominantly children, adolescents and younger adults, survived the acute disorder, but most developed severe neurological syndromes, particularly severe post-encephalitic parkinsonism and other severe motor abnormalities, that incapacitated them for the remainder of their lives. Despite its brief history, encephalitis lethargica played a major role in a variety medical discussions between the two World Wars, as this epitome of neuropsychiatric disease – attacking both motor and mental functions – appeared just as the separation of neurology and psychiatry had reached a critical point.
