Drug Delivery to the Brain

The development of new CNS drugs is notoriously difficult. Drugs must reach CNS target sites for action and these sites are protected by a number of barriers, the most important being the blood–brain barrier (BBB). Many factors are therefore critical to consider for CNS drug delivery, e.g. active/passive transport across the BBB, intra-brain distribution, and central/systemic pharmacokinetics, to name a few. Neurological disease and trauma conditions add further complexity because CNS barriers, drug distribution and pharmacokinetics are dynamic and often changed by disease/trauma. Knowledge of all these factors and their interplay in different conditions is of utmost importance for proper CNS drug development and disease treatment. In recent years much information has become available for a better understanding of the many factors important for CNS drug delivery and how they interact to affect drug action. This book describes small and large drug delivery to the brain with an emphasis on the physiology of the BBB and the principles and concepts for drug delivery across the BBB and distribution within the brain. From the first time that I painted, I had no interest to be a derivative of other artists. My driving intuition was to be influenced nonconsciously by brain insights. My privileged view of the brain, is of overseeing the data patterns of human brain function in the world’s largest brain database. My art is mainly abstract expressionism that reflects my understanding of how the human brain and behavior works. Each series of paintings continue for a few months and are driven by a different inspiration from the nonconscious-conscious dynamics of my brain. The last series in this book: “Unbridled” – is where my nonconscious brain most dominated whatever spontaneously emerged (with no preconceptions) ‘in the moment’, in each painting. I am indebted to the wide influences from my background in brain science, medicine and the international brain database that I set up. The sensory and motor cortical homunculi proposed by Walter Penfield were a major landmark for the anatomical mapping of the brain. More than 60 years after, the development of new tools to investigate brain function non-invasively has increased our knowledge about the structure and functions of the primary motor Cortex (M1) beyond motor control in both humans and animals. This book highlights the role of the motor cortex that goes way beyond motor functioning. We were interested in both theoretical and empirical contributions related to electrophysiological, pharmacological, neuroimaging, and neuromodulatory studies exploring the role of M1 on non-motor functions, such as pain, abnormal neuroplasticity that may lead to chronic pain conditions; or the relationship between M1 and mental imagery or emotion. This book is comprised of 15 articles published in this edited volume as a research topic collection in Frontiers in Human Neuroscience titled “The Role of Primary Motor Cortex as a Marker and Modulator of Pain Control and Emotional-Affective Processing.”
