Natural Orifice Translumenal Endoscopic Surgery (NOTES)

Book Description

Are you interested in using NOTES to treat your patients? Do you need a multimedia tool to guide you through all aspects of clinical management? Just as laparoscopic surgery revolutionized surgical practice in the 1980’s and 90’s, offering genuine competition to traditional open surgery, Natural Orifice Translumenal Endoscopic Surgery (NOTES) presents a genuinely different alternative for surgeons and patients alike as we move forward in the 21st century. Advantages over laparoscopic surgery include lower anesthesia requirements, faster recovery and shorter hospital stays, avoidance of transabdominal wound infections, less immunosuppression, better postoperative pulmonary and diaphragmatic function and of course, the potential for "scarless" abdominal surgery. Combining high-level text with a superb companion website, Natural Orifice Translumenal Endoscopic Surgery (NOTES): Textbook and Video-Atlas provides you with a hands-on, in-depth and practical multimedia tool on this exciting breakthrough, to consult either on the ward or in surgery. Highlights include: Full coverage of the fundamentals of NOTES, its current clinical applications and techniques, and future perspectives Over 50 outstanding videos illustrating NOTES being performed by the experts Full-color throughout and superbly illustrated with over 250 figures Edited by global leaders and pioneers in the field, who have recruited a world-class contributor team As well being a valuable text for gastroenterologists, GI surgeons and general surgeons, Natural Orifice Translumenal Endoscopic Surgery (NOTES): Textbook and Video-Atlas will also appeal to urologists, urologic surgeons, gynecologists, gynecologic surgeons, thoracic and ENT surgeons.

NOTES and Endoluminal Surgery

Book Description

This volume provides a broad introduction in Natural Orifice Transluminal Endoscopic Surgery (NOTES) to the novice, as well as a comprehensive and up-to-date review of the major areas of research in this field for those already performing NOTES. Designed as stand-alone treatises on each particular topic, the chapters are arranged around management of esophageal motor disorders, gastric closure, transgastric NOTES, transvaginal procedures, transrectal NOTES and natural orifice colorectal surgery, and also novel natural orifice techniques that go beyond routine diagnostic and therapeutic endoscopy. Written by experts in the field, NOTES and Endoluminal Surgery is a valuable resource for gastroenterologists and surgeons working in the field of NOTES.

Natural Orifice Transluminal Endoscopic Surgery: The Next Evolution

Book Description

NOTES: Natural Orifice Transluminal Endoscopic Surgery, is a relatively new advancement of surgery and is still in its evolving state. Increasing interest in this surgery comes from the many advantages and better outcomes that it offers as compared to conventional open or other minimally invasive surgery. The book summarizes the evolution, concept, different types, complications, current status and the challenges of NOTES. We also report our results and experience with a new technique of pure transvesical NOTES, with a safe peritoneal entry and bladder closure.

Natural Orifice Transluminal Endoscopic Surgery, an Issue of Gastrointestinal Endoscopy Clinics

Book Description

Since 2000, there has been an impressive and growing body of published data dedicated to natural orifice translumenal endoscopic surgery (NOTES) from all over the world. This issue of the Gastrointestinal Endoscopy Clinics of North America seeks to harness this data and place perspective on the current and future position of NOTES for surgical interventions. The Natural Orifice Surgery Consortium for Assessment and Research (NOSCAR) has been instrumental in organizing, directing, and funding research with a mantra for safe introduction of NOTES to clinical practice. This issue focuses on topics that NOSCAR has touted for safe promulgation of NOTES.

Overcoming Barriers to Natural Orifice Translumenal Endoscopic Surgery (NOTES)

Book Description

Natural Orifice Translumenal Endoscopic Surgery (NOTES) avoids skin incisions by accessing the abdominal cavity through natural orifices. Benefits include less pain, fewer complications and no scars. The aims of this study were to evaluate safety and efficacy of access techniques for natural orifice surgery, to investigate safety and efficacy of closure methodologies and to compare the inflammatory response induced by NOTES with that following conventional surgery. Access techniques were evaluated for safety and efficacy by measuring resultant injury and time required to access the peritoneal cavity in an acute porcine model. Four different gastrotomy closure modalities were evaluated for safety and efficacy by measuring clinical data, evidence of infection and closure integrity in a chronic porcine study. Markers of inflammation were measured in a chronic study comparing NOTES to conventional surgery. 70 anterior transgastric access procedures were performed without any serious injury to adjacent organs. NOTES access required significantly longer than laparoscopic access. Of the seven methods evaluated none was significantly superior to others. In distinguishing between safe and unsafe alternate access sites, endoscopic ultrasound (EUS) provided a statistically significant difference for antral and posterior stomach access points but not for rectal access. Gastrotomy closure time varied widely for the four techniques evaluated. No leak was detected with any method and the strength of closure for each technique was equivalent to control. Three of the four methods resulted in injury and evidence of infection was found in all groups. Postoperative elevation of inflammatory markers was not significantly different between NOTES and laparoscopy except in the case of cortisol, which was greater in the laparoscopic group. This study supports the safety of anterior transgastric access while demonstrating the potential use of EUS in minimizing risks of injury at alternate sites. Likewise, evidence for the reliability of numerous closure techniques was demonstrated with concerns for the safety. Additionally, inefficient access and closure data exposed the technical challenges facing NOTES practitioners. Most importantly, NOTES produced an inflammatory response which was not significantly greater than that produced by conventional laparoscopic surgery, validating its potential and emphasizing the need to overcome the aforementioned technical challenges.

Scar-Less Surgery

Book Description

Laparoendoscopic Single-Site Surgery (LESS) and Natural Orifice Transluminal Endoscopic Surgery (NOTES) are based on the premise that patients should benefit in terms of recovery time, physical discomfort, pain, and cosmesis if a surgical procedure can be performed without visible scarring. Scar-Less Surgery: NOTES, Transumbilical, and Others is a multi-disciplinary effort which weaves together the surgical aspects of these novel techniques. It is the first comprehensive reference on the subject providing state-of-the-art technical information to both general and urologic surgeons while also highlighting future developments and research needs.
