A review of the FAA¿s process for reporting and investigating operational errors. Operational errors (when a controller fails to maintain separation between two aircraft) can be extremely serious incidents that can lead to a catastrophic accident. Ensuring that all events involving a loss of separation are accurately reported, investigated, and addressed is critical to the safe operation of the National Airspace System. The objectives of the review were to: (1) determine whether FAA has adequate policies and procedures to ensure accuracy and consistency in operational error reporting; and (2) review the roles and responsibilities of the Air Traffic Org. and FAA¿s Aviation Safety line of business in reporting and investigating operational errors.
