Household dietary patterns and the cost of a nutritious diet in Myanmar

Despite significant poverty reduction over the past decade, undernutrition in Myanmar remains widespread. Food prices play an important role in influencing diets and nutrition outcomes, especially for poorer households. In this study, we use national household food expenditure data to assess dietary patterns and estimate regional costs of nutritious diets in Myanmar relative to a recommended diet derived from food-based dietary guidelines. We estimate these costs following the cost of a recommended diet method (CoRD), which is based on minimum food group prices. We also develop and demonstrate an extension of this method using food group prices that reflect typical food consumption preferences (CoRD-FP). We assess the affordability of the recommended diet by comparing observed household food expenditure to the CoRD and the CoRD-FP. In 2015, 52 percent of the Myanmar population lived in households with food expenditure below the CoRD-FP, compared to 70 percent in 2010. Even the CoRD, which measures the lowest possible cost of meeting the recommended diet, exceeded household food expenditure for 32 and 24 percent of the population in 2010 and 2015, respectively. Low affordability is driven by high costs of animal-source foods and vegetables, which account for half the CoRD-FP. A majority of households over-consume staples and under-consume micronutrient-dense food groups. This imbalance is driven in part by the high caloric price of nutrient-dense foods relative to rice. The inability of more than half of households in Myanmar to afford a recommended diet at existing food expenditure levels suggests the need for policies that reduce the prices of micronutrient-dense foods, ideally through pro-poor improvements in agricultural productivity and marketing.







Myanmar’s agrifood system: Historical development, recent shocks, future opportunities

Myanmar has endured multiple crises in recent years — including COVID-19, global price instability, the 2021 coup, and widespread conflict — that have disrupted and even reversed a decade of economic development. Household welfare has declined severely, with more than 3 million people displaced and many more affected by high food price inflation and worsening diets. Yet Myanmar’s agrifood production and exports have proved surprisingly resilient. Myanmar’s Agrifood System: Historical Development, Recent Shocks, Future Opportunities provides critical analyses and insights into the agrifood system’s evolution, current state, and future potential. This work fills an important knowledge gap for one of Southeast Asia’s major agricultural economies — one largely closed to empirical research for many years. Global food policy report 2024: Food systems for healthy diets and nutrition

Poverty, food insecurity, and social protection during COVID-19 in Myanmar: Combined evidence from a household telephone survey and micro-simulations

This study assesses the welfare impacts of COVID-19 on households in Myanmar by combining recent high-frequency telephone survey evidence for two specific rural and urban geographies with national-level survey-based simulations designed to assess ex-ante impacts on poverty with differing amounts of targeted cash transfers. The first source of evidence – the COVID-19 Rural and Urban Food Security Survey (C19- RUFSS) – consists of four rounds of monthly data collected from a sample of over 2,000 households, all with young children or pregnant mothers, divided evenly between urban and peri-urban Yangon and the rural Dry Zone. This survey sheds light on household incomes prior to COVID-19 (January 2020), incomes and food security status soon after the first COVID-19 wave (June 2020), the gradual economic recovery thereafter (July and August 2020), and the start of the second COVID-19 wave in September and October 2020. This survey gives timely and high-quality evidence on the recent welfare impacts of COVID-19 for two important geographies and for households that are nutritionally highly vulnerable to shocks due to the presence of very young children or pregnant mothers. However, the relatively narrow geographic and demographic focus of this telephone survey and the need for forecasting the poverty impacts of COVID-19 into 2021 prompt us to explore simulationbased evidence derived by applying parameter shocks to household models developed from nationally representative household survey data collected prior to COVID-19, the 2015 Myanmar Poverty and Living Conditions Survey (MPLCS). By realistically simulating the kinds of disruptions imposed on Myanmar’s economy by both international forces, e.g., lower agricultural exports and workers’ remittances, and domestic COVID-19 prevention measures. e.g., stay-at-home orders and temporary business closures, we not only can predict the impacts of COVID-19 on household poverty at the rural, urban, and national levels, but also can assess the further benefits to household welfare of social protection in the form of monthly household cash transfers of different magnitudes. Combined, these two sources of evidence yield insights on both the on-the-ground impacts of COVID-19 in recent months and the potential poverty reduction impacts of social protection measures in the coming year. We conclude the study with a discussion of the policy implications of these findings.




Food prices and the wages of the poor: A low-cost, high-value approach to high-frequency food security monitoring

International food prices have become increasingly volatile in recent decades, with “global food crises” in 2008, 2011 and most recently in 2022. The 2008 crisis prompted international agencies to ambitiously extend their monitoring of domestic food prices in developing countries to strengthen early warning systems and food and nutrition surveillance. However, food inflation by itself is not sufficient for measuring disposable income or food affordability; for that, one must measure either changes in income or changes in an income proxy. Here we propose the use of a low-cost income proxy that can be monitored at the same high frequency and spatial granularity as food prices: the wages of poor unskilled workers. While not all poor people are unskilled wage earners, changes in the real “reservation wages” of low skilled activities are likely to be highly predictive of changes in disposable income for poorer segments of society (Deaton and Dreze 2002). We demonstrate this by estimating changes in “food wages” – wages deflated food price indices – during well-documented food price crises in Ethiopia (2008, 2011 and 2022), Sri Lanka (2022) and Myanmar (2022). In all these instances, food wages declined by 20-30%, often in the space of a few months. Moreover, in Myanmar we use a household panel survey data to show that the decline in food wages over the course of 2022 closely matches estimate declines in household disposable income and proportional increases in income-based poverty. We argue that the affordability of nutritious food for “all people, at all times” is a critically important dimension of food security, and we advocate for monitoring the wages of the poor as a cheap and accurate means of capturing that dimension.




Dietary quality and nutrition in Myanmar: Past progress, current and future challenges

In the decade prior to the COVID-19 pandemic, Myanmar was in the midst of a dietary transition driven by rapid economic growth and urbanization. In this study, we first use national survey data to compare household diets in 2015 to the healthy diet recommendations of food-based dietary guidelines adapted for Myanmar, as well as estimated nutrient consumption relative to recommended intakes. We use these food group and nutrient consumption gaps to estimate a new measure of multidimensional dietary deprivation developed by Pauw et al. (2022), and a novel extension of that index to nutrient deprivation. Both deprivation indices are strongly negatively correlated with total household expenditure. We then use food demand estimation to estimate income and own price elasticities, which reveal strong preferences for animal-sourced foods, but weaker preferences for vegetables and pulses. Expenditure data also point to strong demand for oils/fats – a problem observed throughout developing Asia (Pingali and Abraham 2022) – and for food away from home, which partially explains the rising burden of overweight/obesity in Myanmar. Moreover, since most nutrient-dense foods are income- and price-elastic, estimated income elasticities suggest that recent declines in household income and increases in food prices in Myanmar will result in declining dietary diversity. We show that this is indeed the case utilizing household phone surveys conducted in recent years. We first use a food vendor survey to show that the cost of a healthy diet increased by 61 percent between September 2021 and September 2022. Next, we analyze a rural Dry Zone panel survey implemented 10 times over 2020-2021 and find that maternal and child dietary diversity both declined significantly as Myanmar’s economic situation deteriorated. Then, in a nationally representative phone survey conducted quarterly in 2022 over a period of rapid food inflation, we find further deterioration in diet quality among adults, but no deterioration among children 6-23 months of age, suggesting parents may be trying to insulate their children from the worst effects of the crisis. Finally, we conclude the paper by discussing policy and program options in very difficult political circumstances. Malnutrition is a multidimensional problem requiring multisectoral solutions, but at present the breakdown in the provision of even basic services makes significant progress highly unlikely, and reversing the recent deterioration in dietary quality and nutrition will surely require resolution of Myanmar’s political crisis. In the interim, we discuss three potentially effective types of interventions: (1) rice fortification to reduce micronutrient deficiencies; (2) homestead food production to improve dietary quality in farm households and rural and peri-urban communities; and (3) nutrition-sensitive social protection for vulnerable mothers and young children, with transfers ideally accompanied by nutrition education interventions.




Rural household welfare in Papua New Guinea: Food security and nutrition challenges

Papua New Guinea continues to encourage a policy focus on food and nutrition security. The PNG National Nutrition Policy (2016-2026) and Nutrition Strategic Action Plan (2018-2022) (NSAP) set a path to improve coordination, secure sufficient funding, and improve technical capacity of nutrition-focused pro gram implementation. As policy prioritizes improved nutrition outcomes, it is important to understand the cost that households face of securing a higher level of nutrition. Ensuring a healthy diet that meets nutrition standards is relatively expensive in PNG. The analysis presented in this paper, which uses detailed household food and non-food consumption data suggests that 4/5 of households in the survey sample live below the healthy diet poverty line (which sets a calorie threshold and defines healthy diet nutrition targets). That is, these households do not have the income available (or do not consume sufficient food and non-food goods) to meet their basic needs which includes securing a nutritious diet that meets food based die tary guidelines.




Cost and affordability of healthy diets across and within countries

Price and affordability are key barriers to accessing sufficient, safe, nutritious food to meet dietary needs and food preferences for an active and healthy life. In this study, the least-cost items available in local markets are identified to estimate the cost of three diet types: energy sufficient, nutrient adequate, and healthy (meeting food-based dietary guidelines). For price and availability the World Bank’s International Comparison Program (ICP) dataset is used, which provides food prices in local currency units (LCU) for 680 foods and non-alcoholic beverages in 170 countries in 2017. In addition, country case studies are developed with national food price datasets in United Republic of Tanzania, Malawi, Ethiopia, Ghana and Myanmar. The findings reveal that healthy diets by any definition are far more expensive than the entire international poverty line of USD 1.90, let alone the upper bound portion of the poverty line that can credibly be reserved for food of USD 1.20. The cost of healthy diets exceeds food expenditures in most countries in the Global South. The findings suggest that nutrition education and behaviour change alone will not substantially improve dietary consumption where nutrient adequate and healthy diets, even in their cheapest form, are unaffordable for the majority of the poor. To make healthy diets cheaper, agricultural policies, research, and development need to shift toward a diversity of nutritious foods.




Affordability of nutritious diets in rural India

Malnutrition is endemic in India. In 2015-16 some 38% of preschool children were stunted and 21% were wasted, while more than half of Indian mothers and children were anemic. There are many posited explanations for the high rates of malnutrition in India, but surprisingly few discuss the role of Indian diets, particularly the affordability of nutritious diets given low wages and the significant structural problems facing India’s agricultural sector. This study was undertaken to address knowledge gaps around the affordability of nutritious diets in rural India. To do so we used nationally representative rural price and wage data to estimate the least cost means of satisfying India-specific dietary recommendations, referred to as the Cost of a Recommended Diet (CoRD), and assess the affordability of this diet relative to male and female wages for unskilled laborers. Although we find that dietary costs increased substantially over 2001-2011 for both men and women, rural wage rates increased more rapidly, implying that nutritious diets became substantially more affordable over time. However, in absolute terms nutritious diets in 2011 were still expensive relative to unskilled wages, constituting approximately 50-60% of male and about 70-80% of female daily wages, and were often even higher relative to minimum wages earned from the Mahatma Gandhi National Rural Employment Guarantee Act (MGNREGA). Since many poor households have significant numbers of dependents and substantial non-food expenditure requirements, it follows that nutritious diets are often highly unaffordable for the rural poor; we estimate that 45-64% of the rural poor cannot afford a nutritious diet that meets India’s national food-based dietary guidelines. Our results point to the need to more closely monitor food prices through a nutritional lens, and to shift India’s existing food policies away from their heavy bias towards cereals. Achieving nutritional security in India requires a much more holistic focus on improving the affordability of the full range of nutritious food groups and ensuring that economic growth results in sustained income growth for the poor.
