Clinicians and researchers observing the natural history of endemic and epidemic infections have always been fascinated by the vagaries of these diseases, in terms of both the changing nature of the disease severity and phenotype over time and the variable susceptibility of hosts within exposed populations. SARS-CoV-2, the virus that causes COVID-19 and is believed to originate from bats, quickly transformed into a global pandemic. The pandemic of the novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has been posing great threats to the global health in many aspects. Currently, there are no proven effective vaccines or therapeutic agents against the virus. Comprehensive understanding of the biology of SARS-CoV-2 and its interaction with hosts is fundamentally important in the fight against SARS-CoV-2. Advanced age, male sex, and comorbidities such as hypertension and cardiovascular disease as well as diabetes and obesity have been identified as risk factors for more severe COVID-19. However, which and to what extent specific genetic factors may account for the predisposition of individuals to develop severe disease or to contract the infection remains elusive. The increasing availability of data from COVID-19 patient populations is allowing for potential associations to be established between specific gene loci and disease severity, susceptibility to infection, and response to current/future drugs.

Co-Evolution Between New Coronavirus (SARS-CoV-2) and Genetic Diversity

The DNA sequences are different between the distinct individuals and these variations produce the species genetic diversity. SARS-CoV-2 virus is a zoonotic SARS-like coronavirus that spreads globally, causing the COVID-19 pandemic disease. The immune response genes are the most various and different in the human genome, correlating with infectious diseases. Genetic variants in the angiotensin-converting enzyme 2 (ACE2) receptor, TMPRSS2, HO-1, BCL11A, and CYP2D6 are predicted to either encourage or inhibit the interaction with the viral proteins and subsequently contribute to coronavirus genetic risk factors. The genetic susceptibility to SARS-CoV-2 was investigated by analyzing different genes,Äô polymorphisms such as ACE2 and TMPRSS2, HO-1, and BCL11A. A specific genetic susceptibility to COVID-19 was found through different populations in TMPRSS2, ACE2, HO-1, and BCL11A genes. Particularly, ACE2 gene polymorphisms were shown to be correlated with pulmonary and cardiovascular conditions by modifying the angiotensinogen-ACE2 system, which recommends the possible explanations of COVID-19 susceptibility based on genetic diversity. Moreover, the COVID-19 treatment could be complicated by such genetic polymorphisms. In conclusion, a good characterization of functional polymorphisms and the host genetics can assist in identifying the pathophysiology of the disease pathway to stratify the risk evaluation and to personalize the treatment procedures.

Immunogenomics/immunopharmacogenomics, a genetic characterization of the immune system made possible by next-generation sequencing (NGS), will be important for the further understanding of the pathogenesis of various disease conditions. Abnormal immune responses in the body lead to development of autoimmune diseases and food allergies. Rejection of recipient cells and tissues, as well as severe immune reactions to donor cells, is also the result of uncontrolled immune responses in the recipient body. There have been many reports indicating that activated immune responses caused by the interaction of drugs and HLA are present in drug-induced skin hypersensitivity and liver toxicity. The importance of the host immune responses has been recognized in cancer treatments, not only for immunotherapy but also for cytotoxic agents and molecular targeted drugs. Hence, characterization of the T-cell receptor and B-cell receptor repertoire by means of NGS deep sequencing will ultimately make possible the identification of the molecular mechanisms that underlie various diseases and drug responses. In addition, this approach may contribute to the identification of antigens associated with the onset or progression of autoimmune diseases as well as food allergies. Although the germline alterations and somatic mutations have been extensively analyzed, changes or alterations of the immune responses during the course of various disease conditions or during various treatments have not been analyzed. It is also clear that computational analyses to draw meaningful inferences of functional recognition receptors on the immune cells remain a huge challenge.
