The Psychobiology of Bulimia Nervosa

Research and Treatment in the Psychobiology of Bulimia Nervosa 1 2 K. M. Pirke and W. Vandereycken When we were preparing this book, the main purpose was to gather the latest in sights from both basic and clinical research in bulimic disorders. The burgeoning scientific interest in eating disorders forced us to restrict the focus in such a way that attention was only paid to psychobiological aspects of (disturbed) nutrition and be havior. This implied that other important aspects, like sociocultural and familial fac tors, had to be excluded. But it turned out that even with such a restricted scope the subject was not an easy one. A review of the contributions to this volume clearly shows that our present understanding of bulimia nervosa is still very small. Reviewing the hypothalamic and especially neuroendocrine regulation of nutrient balance, Bray has emphasized the importance of the autonomic nervous system in regulating food intake and energy expenditure. The role of insulin, adrenal steroids, gonadal steroids, and growth hormone in modulating nutrient intake and storage were discussed. The studies by Jimerson et aI. , Schweiger et aI. , and Fichter et al. show that all these factors are altered in bulimic patients, indicating that the whole system of regulating food intake and storage is severely disturbed in bulimia nervosa. Evidence has been presented that neurotransmitter alterations may occur in bulimia: Jimerson et al.




Perception of Self in Emotional Disorder and Psychotherapy

One's view of self has pervasive and significant effects socially, psychologi cally, and even biologically. Regardless of theoretical differences, most psycho therapists agree that perception of self in one way or another profoundly impacts emotional satisfaction, behavioral adaptation, and rational thinking. Self-accep tance has played a major role in almost every major theory of personality. Despite its recognized importance over the years, only recently has the percep tion of self received vigorous research attention as a central variable in the development and maintenance of psychological dysfunction and as a mediating mechanism in effecting psychological change. Several lines of evidence point to the importance of self-perception in emotional disorder and psychotherapy. Feelings of inadequacy and unworthiness are frequently cited complaints among persons seeking psychological help. Peo ple with low self-esteem see themselves as helpless and inferior. They feel incapable of improving their situation. They fail to evidence the requisite inner resources or coping abilities for tolerating the stress of their life situation. The ability to be involved in healthy intimate relationships, to engage in successful career performance, to experience satisfactory sexual functioning, or to maintain effective mood management are all subject to disruption as a result of inconsis tent and impaired self-appraisal.

Handbook of Clinical Behavior Therapy with Adults


Book Description

Despite the occasional outcries to the contrary, the field of behavior therapy is still growing, and the asymptote has not been reached yet. The umbrella of behavior therapy continues to enlarge and still is able to encompass new theories, new con cepts, new research, new data, and new clinical techniques. Although the number of new behavioral journals now has stabilized, we still see a proliferation of books on the subject. In the past few years, however, we have seen considerable specialization within behavior therapy. No longer is it possible to be a generalist and remain fully abreast of all the relevant developments. Thus, we see behavior therapists who deal with adults, those who deal with children, those whose specialty is hospital psychiatry, and those who see themselves as practitioners of behavioral medicine. Even within a subarea such as behavioral medicine, specialization runs supreme to the extent that there are experts in the specific addictions, adult medical problems, and child medical problems. Given the extent of specialization, there are numerous ways "to skin" the pro verbial "cat." We therefore have chosen to look at the contemporary work in behavior therapy that is being carried out with adults, in part, of course, because of our long-standing interest in this area as teachers, researchers, and clinicians. In so doing, we have chosen to highlight the clinical aspects of the endeavor but not at the expense of the rich research heritage for each of the specific adult disorders.
