Series on Chemical Accidents OECD Guiding Principles for Chemical Accident Prevention, Preparedness and Response - Third Edition

Book Description

Chemical accidents with serious consequences continue to happen in OECD Member countries and worldwide. Over the past decades, successive major accidents have caused deaths, injuries, significant environmental pollution and massive economic losses – from the hydrogen fluoride leak in Gumi (Korea) in 2012, the ammonium nitrate explosion in West, Texas (United States) in 2013 or, recently, the blow-up of a chemical facility in Tarragona (Spain) and the explosion at the port of Beirut (Lebanon) in 2020, and the blast in Leverkusen (Germany) in 2021. This third edition of the OECD Guiding Principles for Chemical Accident Prevention, Preparedness and Response provides guidance for the safe planning and operation of hazardous installations. It aims to support public authorities and industry in taking appropriate actions to prevent chemical accidents and to mitigate impacts of accidents that do nevertheless occur. These guiding principles apply to fixed installations at which hazardous substances are produced, processed, handled, stored, used or disposed of, in such a form and quantity that there might be a risk of occurrence of a chemical accident. These guiding principles constitute the technical guidance supporting the implementation of the Decision-Recommendation of the Council concerning Chemical Accident Prevention, Preparedness and Response adopted in 2023.

Guidelines for Integrating Management Systems and Metrics to Improve Process Safety Performance

Book Description

This book combines the synergies between performance improvement systems to help ensure safe and reliable operations, streamline procedures and cross-system auditing, and supporting regulatory and corporate compliance requirements. Many metrics are common to more than one area, such that a well-designed and implemented integrated management system will reduce the load on the Process Safety, SHE, Security and Quality groups, and improve manufacturing efficiency and customer satisfaction. Systems to improve performance include: process safety; traditional safety, health and environment; and, product quality. Chapters include: Integrating Framework; Securing Support & Preparing for Implementation; Establishing Common Risk Management Systems – How to Integrate PSM into Other EH; Testing Implementation Approach; Developing and Agreeing on Metrics; Management Review; Tracking Integration Progress and Measuring Performance; Continuous Improvement; Communication of Results to Different Stakeholders; Case Studies; and Examples for Industry.

Guidelines for Defining Process Safety Competency Requirements

Book Description

This Guideline presents the framework of process safety knowledge and expertise versus the desired competency level in a "super-matrix" format, vertically and diagonally. The matrix references for potential remedies/required training may be tailored to a company's internally developed training, reference externally available training, or some combination of the two. Chapters include: Identify Process Safety Roles & Competency Needs; Process Safety Competency Matrix; Individual and Corporate Process Safety Competencies; Conduct Assessments vs. Needs; Develop Gap Closure Plans; and Sustaining Competencies.

Guidelines for Implementing Process Safety Management

Book Description

The 2nd edition provides an update of information since the publication of the first edition including best practices for managing process safety developed by industry as well as incorporate the additional process safety elements. In addition the book includes a focus on maintaining and improving a Process Safety Management (PSM) System. This 2nd edition also provides "how to information to" determine process safety performance status, implement one or more new elements into an existing PSM system, maintain or improve an existing PSM system, and manage future process safety performance.

Guidelines for Risk Based Process Safety

Book Description

Guidelines for Risk Based Process Safety provides guidelines for industries that manufacture, consume, or handle chemicals, by focusing on new ways to design, correct, or improve process safety management practices. This new framework for thinking about process safety builds upon the original process safety management ideas published in the early 1990s, integrates industry lessons learned over the intervening years, utilizes applicable "total quality" principles (i.e., plan, do, check, act), and organizes it in a way that will be useful to all organizations - even those with relatively lower hazard activities - throughout the life-cycle of a company.

Risk Analysis and Control for Industrial Processes - Gas, Oil and Chemicals

Book Description

Risk Analysis and Control for Industrial Processes - Gas, Oil and Chemicals provides an analysis of current approaches for preventing disasters, and gives readers an overview on which methods to adopt. The book covers safety regulations, history and trends, industrial disasters, safety problems, safety tools, and capital and operational costs versus the benefits of safety, all supporting project decision processes. Tools covered include present day array of risk assessment, tools including HAZOP, LOPA and ORA, but also new approaches such as System-Theoretic Process Analysis (STPA), Blended HAZID, applications of Bayesian data analytics, Bayesian networks, and others. The text is supported by valuable examples to help the reader achieve a greater understanding on how to perform safety analysis, identify potential issues, and predict the likelihood they may appear. - Presents new methods on how to identify hazards of low probability/high consequence events - Contains information on how to develop and install safeguards against such events, with guidance on how to quantify risk and its uncertainty, and how to make economic and societal decisions about risk - Demonstrates key concepts through the use of examples and relevant case studies

Process Safety

Book Description

Effective process safety programs consist of three interrelated foundations—safety culture and leadership, process safety systems, and operational discipline—designed to prevent serious injuries and incidents resulting from toxic releases, fires, explosions, and uncontrolled reactions. Each of these foundations is important and one missing element can cause poor process safety performance. Process Safety: Key Concepts and Practical Approaches takes a systemic approach to the traditional process safety elements that have been identified for effective process safety programs. More effective process safety risk reduction efforts are achieved when these process safety systems, based on desired activities and results rather than by specific elements, are integrated and organized in a systems framework. This book provides key concepts, practical approaches, and tools for establishing and maintaining effective process safety programs to successfully identify, evaluate, and manage process hazards. It introduces process safety systems in a way that helps readers understand the purpose, design, and everyday use of overall process safety system requirements. Understanding what the systems are intended to achieve, understanding why they have been designed and implemented in a specific way, and understanding how they should function day-to-day is essential to ensure continued safe and reliable operations.
