Strategic Framework for Elimination of Human Rabies Transmitted by Dogs in the South-East Asia Region

Dog bites are the primary source of human infection in rabies-endemic countries and account for 96% of rabies cases in the South-East Asia Region. Elimination of human rabies is dependent on elimination of dog rabies. Some countries have a comprehensive rabies control program but it is a neglected area in others. New innovative tools and techniques have been developed in recent years to improve dog vaccination coverage, dog population management and accessibility of modern rabies vaccines. Considering the importance of consolidating achievements in rabies control in Member countries, the WHO Regional Office for South-East Asia has developed a regional strategy for elimination of human rabies transmitted by dogs in the Region. The strategy aims to eliminate human rabies through progressive control of dog rabies and human rabies prophylaxis in rabies-endemic countries and to maintain the rabies free status in rabies-free areas of the South-East Asia Region by 2020.




Towards Elimination of Dog Mediated Human Rabies


Book Description

Rabies is an ancient zoonotic viral disease that still exerts a high impact on human and animal health. The disease is almost 100% fatal after clinical signs appear, and it kills tens of thousands of people per year worldwide, particularly in Africa and many parts of Asia. Although the disease in humans can be prevented by timely post-exposure prophylaxis, its access and affordability is limited in rabies endemic countries. With 99% of infections in humans caused by rabid domestic dog bites, controlling the infection in this reservoir population has been proven to be most effective to reduce and eliminate human rabies cases. In this context, this Research Topic invited contributions on the control and elimination of dog mediated human rabies. Publications on epidemiological, educational, policy-related and economic aspects of dog and human rabies surveillance, implementation of control in dogs and humans and scientific documentation of success stories were consolidated. We hope that these articles contribute to reaching the ambitious goal, set by key players in global health, of the elimination of dog mediated human rabies by 2030.

Neglected Tropical Diseases - East Asia

Neglected tropical diseases (NTDs) are a group of diseases frequently found in impoverished communities in tropical and sub-tropical countries. The risk for many of the NTDs is high in both deprived urban and rural areas of East Asia. Adapted to the endemic settings and characteristics of the diseases, a range of tools and strategies are currently being rolled out for the large-scale control of many NTDs. Both vector control measures and community sensitization programmes have for example been used to control dengue in urbanized settings. Challenges posed by yaws and lymphatic filariasis are being addressed by mass drug administration, while rabies requires the involvement of the veterinary public health sector for disease control. For leprosy, an elimination target has been defined; however, achieving this goal remains a considerable challenge. Food-borne trematodiases, on the other hand, are emerging and require a deeper understanding of its burden in East Asia and how these diseases can be tackled in a cost-effective manner. Finally, factors, such as an increase of non-communicable diseases due to changing lifestyles which accompany economic growth, the spreading HIV epidemic as well as climate change and the occurrence of natural disasters can potentially affect the epidemiology and control of NTDs. This volume discusses the mentioned topics in detail with contributions by experts in the respective research areas from different working environments.




Rabies in Man and Animals

Rabies is the deadliest zoonotic disease that threatens humans and animals on all continents except Antarctica. Asia and Africa are worst affected as more than 95 per cent of rabies associated human deaths occur in these regions. India alone bears about 36 per cent of the global human rabies burden. Dogs are the main transmitters of rabies that potentially threaten over 3 billion people in Asia and Africa. Many developed nations have been able to successfully control dog rabies but continue to face the risk from wildlife including bats. Bat rabies is responsible for most human rabies deaths in the United States of America and Canada but has emerged as a public health threat in Australia, Latin America and Western Europe as well. Despite being vaccine preventable, rabies continues to haunt the mankind. Poor resources is a major constraint but the factors like low priority attributed to rabies, misconceptions in the community about the disease and unsystematic approach for its prevention and control are also responsible for the grim situation. Targets have been set for elimination of human and dog rabies in all Latin American countries by 2015 and of human rabies transmitted by dogs in South-East Asia by 2020. However, the myths prevailing among the people together with inadequate knowledge of the health professionals, veterinarians, and the civic body staff about the rabies prevention and control strategies might make the task very difficult. This book comprising eight chapters elaborates the causation of rabies in man and animals, its global epidemiology, risk analysis and effective strategies for the management of exposures. Proven methods of rabies prevention and control have been discussed in length along with the challenges and ways to overcome the constraints through intersectoral coordination. The 8th Asia Pacific workshop on multisectoral collaboration at the animal-human-ecosystems interface was held in Bangkok, Thailand from 9 to 11 April 2019. The Food and Agriculture Organization of the United Nations (FAO) hosted the workshop in collaboration with the Regional Tripartite, which included FAO, World Organisation for Animal Health (OIE) and the World Health Organization (WHO). A total of 142 participants attended this workshop including representatives from national governments of 28 countries in the Asia Pacific region, regional organizations, academic institutions, experts and development partners working on human health, animal health and wildlife issues.
