Exercise testing is widely used all over the world to assess functional capacity in athletes, healthy subjects and patients. According to recent surveys, the interest in ergometry is still growing in almost all fields of medicine, especially in private practice. Furthermore, there has been an exponential growth in the number of publications on exercise testing in the last years. Several consensus and task force conferences have dealt with exercise testing and published recommendations on standardization and guidelines in ergometry. These factors have, in combination, initiated an upsurge in research and clinical use of exercise testing. At the 6th International Seminar on Ergometry the latest findings and advances in ergometry were discussed. Reviews and results of the congress covering a wide range of features in exercise testing are presented in this book. The editors hope that this book will make a substantial contribution to our knowledge regarding exercise testing and will help physicians to appropriately evaluate exercise testing in healthy and diseased subjects. This history traces the development of helicopters in the Marine Corps from 1962 to 1973 and is the second in a series of two volumes which between them cover the story of Marines and helicopters from 1946 to the present. In the period covered by this volume, the Marines at last acquired helicopters fully capable of carrying out an amphibious vertical assault, and they further elaborated their helicopter doctrines and tactics. In the Vietnam war, pilots and machines met and surmounted the test of actual combat. The documentary basis for this monograph was primarily the official records of the Marine Corps and Navy Department, but considerable use was made of interviews and correspondence with key individuals involved in all phases of helicopter development. One of the most pervasive characteristics of man is hindsight. It masquerades under many guises: Mon day morning quarterbacking, second guessing, and historical writing. When viewed through time, the past becomes distorted. Problems seem simpler, the choices more clear, and the conditions less complex than those of the present. The men who played a part become more heroic or more villainous than they were in life. This volume is an attempt to portray accurately the difficulties faced and the obstacles conquered by the men who developed helicopters in the Marine Corps, so that the Marines of today and the future may meet the challenges of their own times with the same dedication as their predecessors. The men who developed helicopters in the Marine Corps had nothing more to rely on than their knowledge of what had preceded them, intelligence liberally used, and both mental and physical courage. The present-day Marine will be well served if he applies nothing more.

The biotechnological advances of recent years have put us on the brink of unprecedented gains in animal productivity. Manipulation of animal growth rate and composition of gain is now possible by a variety of techniques. Ex amples include ingestion of beta-adrenergic agonists, injection of somatotropin, castration, immunization, and gene insertion. Animal Growth Regulation ad dresses modem concepts of growth regulation with an emphasis on agricul turally important animals. This emphasis is not exclusive, as many situations exist in which the only information available was generated in other species, and this information has been included for the sake of clarity and completeness. However, because of the overall orientation of this volume, particular attention has been given to the regulation of skeletal muscle, adipose tissue, and bone growth. Certain hormones and growth factors have a profound influence on growth regulation and this basic physiological knowledge is being harnessed to maniplilate growth. Thus, considerable emphasis has been given to growth hor mone-somatomedinlinsulinlike growth factor regulation of cell and tissue growth. The involvement of peptides coded by protooncogenes and of negative growth regulators, such as transforming growth factor-l3, represents an emerging area of molecular biology wherein basic knowledge offers potential exploitation for growth manipulation. Opportunities also exist for regulation of protein turn over, especially from the standpoint of protein degradation. Therefore, a place was reserved for these topics in order to provide relevant basic knowledge.

Development and Implications of Antimicrobial Resistance One of the most ominous trends in the field of antimicrobial chemotherapy over the past decade has been the increasing pace of development of antimicrobial resistance among microbial pathogens. The hypothesis that man can discover a magic bullet to always cure a particular infection has proved false. Physicians are now seeing and treating patients for which there are few therapeutic alternatives, and in some cases, none at all. Until recently there was little concern that physicians might be losing the war in our ability to compete with the evolving resistance patterns of microbial pathogens. Now the general public is very aware of the threat to them if they become infected, thanks to cover story articles in major magazines such as Time, Newsweek, newspapers, and other news sources. Antimicrobial resistance is not a novel problem. Shortly after the widespread introduction of penicillin in the early 1940s, the first strains of penicillin-resistant staphylococci were described. Today it is an uncommon event for a clinical laboratory to isolate an S. aureus that is sensitive to penicillin. Other gram-positive strains of bacteria have become resistant, including the exquisitely sensitive Streptococcus pneumoniae. Sensitivity to vancomycin was once so uniform that it was used in routine clinical laboratories as a surrogate marker for whether an organism should be classified as a gram-positive. That criterion can no longer be relied upon because of emerging resistance among some species. Gram-negative bacteria, viruses, fungi, and parasites all have succeeded in developing resistance.
